Dear Paxton Consolidated Schools Students and Parents/Guardians:
We are so excited to welcome you to Paxton Consolidated Schools for the 2023-2024 school year!
In an effort to streamline the yearly, required student and parent acknowledgement and agreement
signatures, we have combined several policies, documents and forms into a single document for you
to sign and return to the school office by August 25, 2023!
Please carefully review the enclosed, acknowledgement and receipt form.
By signing the form, you, (the student and his/her parent/guardian), acknowledge that you have read
and agree to follow the Paxton Consolidated Schools guidelines, policies and terms set forth within:

The Paxton Consolidated School District does not provide any type of health or accident
insurance for injuries incurred by your child at school. We encourage all families to have
accident coverage for their children, prior to participation in any interscholastic sports or if a
family’s current primary health insurance has a high deductible, Co-Insurance Clause and/or
limitation on medical benefits. If you feel your primary health coverage is adequate, please
check the DO NOT wish to participate box on the insurance question on the form. If interested,
follow the directions on the insurance form. ( www.sas-mn.com )

Please review the three choices outlined on the form regarding the image release for
your student for the 2023-2024 school year.
